EMERGENCY MEDICAL TECHNICIAN (EMT-B) HYBRID
ENROLLMENT AGREEMENT
1. I acknowledge that the school has the right to adjust program content, materials, or
schedules as deemed necessary. The school also reserves the right to terminate my training
for reasons such as unsatisfactory progress or attendance, non-payment of tuition or fees,
or failure to comply with the school's policies and procedures.
2. I understand that my tuition charges cover the right to attend classes in which I am enrolled
and are not contingent upon my academic progress, personal satisfaction, or attainment of
employment upon graduation. I further acknowledge that violating any policies, rules, or
regulations of American Safety Programs & Training may result in forfeiture of my rights
to attend class and the school.
3. I am aware that I am responsible for tuition and fees related to the program's required
course of study. The current tuition rate remains applicable as long as I am continuously
enrolled and complete the program, as approved at the time of admission. If I change
programs or withdraw from school and subsequently re-enroll, I am required to sign a new
enrollment agreement/contract with the tuition rate effective on the date of re-enrollment.
4. I have read and comprehended the Enrollment Agreement, acknowledging receipt of an
exact copy. I understand that this Agreement encompasses all the terms of my enrollment,
and I affirm that no verbal statements contrary to its contents have been made.
5. I acknowledge the following:
a. By signing this agreement, I understand that I am enrolled in the EMT Basic Hybrid
program at American Safety Programs and Training, Inc. which I have access to for
12 months. At the conclusion of this program, I will receive a Certificate of
Completion that will permit me to apply to take the NREMT Exam.
b. The tuition for the EMT Basic Hybrid program is $1,750. American Safety
Programs & Training, Inc. accepts the following payment methods for all the
courses it offers: Cash; Credit/Debit Card; Money Order; Certified Check and
IGW Financing.
Included in Tuition:
1. Access to Jones & Bartlett Learning (JBL) Management System
2. American Heart Association's Basic Life Support certification

Not Included in Tuition:
1. State Examination Fees:
a. Upon passing the course, students will be eligible to sit for the NREMT cognitive
exam.
b. This exam is administered by Pearson VUE.
c. The cost to sit for the state licensing exam is $104.
2. EMT-B State-Issued License:
a. The Rhode Island Department of Health manages licenses for all EMTs. Upon
passing the NREMT exam, students will receive their NREMT registration card to
obtain a license from the EMS office in the state they plan to practice.
b. The cost for the Rhode Island EMT-B license is $120.
c. The cost of the EMT-B license varies depending on the state of residence.
c. This EMT Basic Hybrid program begins on and concludes on
am enrolling in the self-paced program which I understand I have one year to complete.
d. Refund Policy: If you enroll in a class at American Safety Programs & Training, Inc. but
need to withdraw for any reason, the first step is to send an email to the below email
addresses. Your intent to withdraw from a class or cancel any IGW contract is not valid
until this email is sent.

In the event you register for a class but do not attend any classes, there is a $100
cancellation fee to withdraw. This $100 cancellation fee is also assessed for students with
an IGW Solutions contract. No contract will be canceled with IGW until the cancellation
fee is paid.
Once you start attending classes, the refund policy is calculated on a prorated basis.
a. Every EMT-B Hybrid program student must pay the $300 deposit which is non-
refundable.
b. For example, if you are in the 1-year (365 days) EMT-B Hybrid course, the total
cost is $1,750, meaning each day costs $4.79 after the $300 deposit is received.
c. If you withdraw from the course on day 125, you are responsible for paying for
125 days of access (125 days x $4.79 = $598.75), plus an additional $100
cancellation fee.
d. In the above example, the total amount the student would be responsible for
paying is $698.75.
e. The above-referenced $698.75 needs to be paid in full before any IGW contract
is canceled.
Prorated refunds are based on the class schedule, not the number of payments in your
IGW plan.

3. By signing this contract, I acknowledge that I have read the contract/handbook, understand
the terms and conditions, and agree to the outlined conditions. It is further understood that
this agreement supersedes all prior verbal or written agreements and cannot be modified
without the written agreement of both the student and the School Official. Both the student
and the school will retain a copy of this agreement.
