Release of Liability and Assumption of Risk Agreement
By participating in any course, program, or training offered by American Safety Programs & Training, Inc., located at 150 Niantic Ave, Providence, RI 02907, I hereby acknowledge and agree to the following:
I fully understand and acknowledge that there are inherent risks and hazards associated with participation in training courses offered by American Safety Programs & Training, Inc. These risks may include, but are not limited to, personal injury, illness, permanent disability, or death, as well as loss of or damage to personal property.
I hereby knowingly and voluntarily assume all such risks, whether known or unknown, and fully accept personal responsibility for any resulting injury, loss, or damage.
Furthermore, I, for myself, my heirs, executors, administrators, and assigns, fully release, waive, discharge, and hold harmless American Safety Programs & Training, Inc., its officers, employees, instructors, agents, and affiliates, from any and all claims, demands, liabilities, actions, causes of action, damages, costs, or expenses (including attorney’s fees) arising out of or related to any injury, disability, death, or loss or damage to person or property that may occur as a result of my participation in any training course, including but not limited to any negligence on the part of the company or its representatives.
This release applies to any course commencing on or after the date below and remains in effect indefinitely unless revoked in writing.
